Important information before your first visit:

If you are planning on using insurance to cover the cost of your visit, please verify you have acupuncture benefits- including your co-pay. This information can be found in your handbook or oftentimes by looking on your portal or calling the number on the back of your card. There is a list of companies I am in network with on my website, and if you don’t see your insurance listed, you can check to see if you have out of network coverage. If you’d like us to verify your benefits, please give us at least 48 hours notice (not including weekends).

To verify benefits we need your name, -as it is on your health insurance card, DOB, and address-that is on file with your health insurance company.

For PacificSource Community Solutions patients, you do not have to get your benefits verified. You have acupuncture/chiropractor benefits and I am in network. You receive 20 visits combined acu+chiro per calendar year. You do not need a medical doctor to refer you.

If we don’t verify your benefits before your visit, we require you to pay out of pocket for your visit.

We will not charge your credit card (required to place on file) unless you have a co-pay or you purchase something from the clinic.
